Kindly submit the following documents along with your bid on or before the closing date, August 18, 2025, at 10:00 AM:

1. PhilGEPS Certificate of Registration
2. Updated Mayor-s/Business Permit
3. Notarized Omnibus Sworn Statement (for amounts Php 50,000 and above)
4. Latest Annual Income Tax Return (for amounts Php 500,000 and above)
5. Tax Clearance Certificate in accordance with BIR RR No. 17-2024
6. BIR Form 2303

Please be informed that, as part of the technical requirements, a food tasting will be held on August 18, 2025, from 11:00 AM to 12:00 NN at the Office of the Solicitor General, 134 Amorsolo Street, Legaspi Village, Makati City.

Suppliers are required to provide at least two (2) sample sets for tasting and technical evaluation (particularly for quality assessment) by the agency-s authorized representatives.
