Delivery Period: 30 days upon receipt of Purchase Order

Interested Service Provider shall submit their filled-up Request for Quotation (RFQ) to this Office (walk-in or email)
together with the updated copy of the following requirements:

1. Mayor's/Business Permit;
2. PhilGEPS Registration Number/Certificate; and
3. Income/Business Tax Return

The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 5:00PM of February 25, 2025.
Opening of Quotations will be held on PENRO Conference Room, 10:00 AM of February 26, 2025.
