Description : Other Conditions:
1. Deliveries must be from fresh commercial stock with expiry dates not less than twenty-four (24) months from
the date of delivery;
2. Items with physical signs of degradation such as discoloration of tablets, capsules, solutions and powder for
reconstitution, chips, cracks in tablets, crushed, caking in suspensions and moisture droplets in vials of dry
powder shall be returned to and replaced by the supplier at no additional cost. Likewise, medicine with packaging
that have signs of damage of deterioration shall be returned to the supplier;
3. A valid Certificate of Product Registration must accompany every delivery with Food and Drug Administration
(FDA) Batch Certificates;
4. Each bottle/box/tin foil must bear in print on the label the following upon delivery:
 The generic name including the salt and the same should appear in the insert, prominently displayed,
followed by the brand name, if any, in small print, enclosed in parenthesis;
 The registration number with FDA;
 The lot number, batch or control number; and
 The expiration date including the mo...
