  Attach proof of compliance of the following conditions:
  
  1. The bidder must be accredited by DOH for the conduct of drug screening tests.
  2. The bidder must have a history of similar drug testing contracts with a government agency.
  3. The bidder must have a minimum of 5 years of experience in the industry.
  4. The bidder must submit an affidavit of undertaking to shoulder the expenses for the confirmatory drug testing for individuals who tested positive in the screening test.
  5. The bidder must capable of providing on-site test results.
  6. The bidder must be capable of delivering and conducting drug screening tests for public utility vehicle (PUV)
  drivers of all ages during the following annual field operations, upon request and in accordance with a schedule
  approved by the LTO Assistant Secretary:
