1.Valid Mayor's Permit
2.Valid PhilGEPS Certification or PhilGEPS Registration Number
3.Omnibus Sworn Statement, accompanied by the duly notarized Special Power of Attorney (SPA), Board/Partnership Resolution, or Secretary-s Certificate, whichever is applicable
4.Latest Income Tax Return (ITR) filed through eFPS/eAFS with proof of confirmation receipt and proof of payment (if applicable) through BIR-authorized agent bank.

Failure to submit said documents will be a ground for disqualification.
Quotations for each item should not exceed the budget for each item
