Interested Service Provider shall submit their filled-up Request for Quotation (RFQ) to this Office (walk-in or email)
together with the updated copy of the following requirements:

1. Mayor's/Business Permit;
2. PhilGEPS Registration Number/Certificate; and
3. Income/Business Tax Return

The Omnibus Sworn Statement may be submitted together with the RFQ Form or prior to the issuance of award.

Submission of Quotation will be on or before 12:00PM of February 27, 2025.
Opening of Quotations will be held on PENRO Conference Room, 2:00 PM of February 27, 2025.
