TERMS OF REFERENCE
I. PROJECT AND LOCATION

PROJECT: SUPPLY AND DELIVERY OF CELLCARDS AND ELOAD FOR TWELVE (12) MONTHS CONSUMPTION

LOCATION: PHILIPPINE COCONUT AUTHORITY DILIMAN, QUEZON CITY

II. SCOPE OF WORK

The work to be performed shall be:

1. Supply and delivery of the following Cellcards and E-load:

• 171 pcs Smart Prepaid card, 500
• 11 pcs Smart Prepaid card, 300
• 36 pcs Smart Prepaid card, 100

• 24 pcs Globe E-load, 1000
• 12 pcs Globe E-load, 300
• Globe E-load, (worth 6,288.00)

2. Delivery and payment of cellcard shall be on a monthly basis

3. ALL REGULAR LOADS ONLY. (Note: Expiration should atleast one (1) year from the date of delivery.

TOTAL ABC: ₱ 125,788.00

III. OTHER TERMS AND CONDITIONS

1. The prospective bidder shall submit the following together with the proposal in a
sealed envelope on March 5-10, 2025 9:00 am at 5th floor, General Services Division,
PCA R&D Building. Diliman, Quezon City.
