GUIDELINES/ INSTRUCTIONS TO BIDDERS
A. Only SEALED QUOTATION (in your own format) received in the Procurement Unit, LTO -Central Office, East
Avenue, Quezon City shall be accepted. E-mailed quotation/s shall not be entertained. Submit your quotation
together with the following documents:
1. Valid Mayor's/Business Permit indicating the line of business.
2. PhilGEPS Reg. Number
3. Valid DTI or Securities and Exchange Commission (SEC)
4. If Corporation, submission of Notarized Secretary's Certificate is required.
5.Original Notarized Omnibus Sworn Statement in accordance with R.A 12009 (submission of Original Notarized
Omnibus Sworn Statement in accordance with R.A 9184 shall be considered as non-responsive)
Incomplete submission of the above-mentioned requirements are considered NON-RESPONSIVE.
