• Blood Chemistry to include: Fasting Blood Sugar (FBS), SGPT, Lipid Profile (Triglycerides, HDL, LDL, VLDL), Blood Urea Nitrogen (BUN), Creatinine, Uric Acid
2. Hba1C
• For EEs diagnosed with diabetes
• For EEs identified as having increased risk of type 2 diabetes
• For EEs with symptoms of diabetes
3. Breast Mammogram for females 40 years old and above
4. Pap Smear for females 35 years old and above and married females regardless of age
5. Prostate Specific Antigen (PSA) for males 40 and above

The private health care facility/service provider engaged to do the diagnostic and screening procedures/physical examinations must meet the following:
1. Must have fully functional equipment (i.e., X-ray, ECG machine, blood chemistry analyzer, etc.).
2. Must have clean and well-maintained laboratory clinic or mobile facilities.
