BLOWER AT TB CENTER FOR INE (1) YEAR
THE SERVICE PROVIDER OBLIGATION:
1. CLEANING OF INDOOR AND OUTDOOR (QUARTERLY);
2. HEPA FILTER REPLACEMENT (SEMI-ANNUAL);
3. EXHAUST FILTER REPLACEMENT (SEMI-ANNUAL);
4. ON CALL SERVICES FREE OF CHARGE 24/7 RESPONSE TIME;
5. PROPER DISPOSAL OF HEPA FILTER AND OTHER WASTE
MATERIAL WILL BE IN FULL RESPONSIBILITY OF THE SERVICE PROVIDER;
6. MONTHLY REGULAR CHECK-UP OF ALL UNITS, INCLUDING
TROUBLESHOOTING OF ALL DAMPERS, SENSOR, FIXTURES AND
POWER SUPPLY, DUCTING AND WIRINGS;
7. PROVIDE STANDARD HEPA FILTER AND EXHAUST FILTER
INCLUDING INSTALLATION/REPLACEMENT;
8. IN CASE SUPPLIER IS OUTSIDE COTABATO CITY, SERVICE
TECHNICIAN MUST BE ON-CALL/STAND-BY FOR IMMEDIATE ANY
TIME OF THE DAY;
9. TO SUBMIT SCHEDULE OF ACTIVITIES SCOPE OF WORK TO BE DONE;
10. SUBMIT LIST OF CONSUMABLES W/ NUMBER OF QUANTITIES,
SPECIFIC BRAND AND COUNTRY OF ORIGIN;
