3. Submit duly notarized Omnibus Sworn Statement supported by:
a) For sole proprietorship where the general manager or the sole proprietor himself submits the bid for the company, a duly notarized Affidavit stating that the fact of his representation in the company, and that he posseses the authority to represent and bind the same. If a person other than the general manager or sole proprietor submits a bid for or represents the company, a duly notarized Special Power of Attorney delegating such authority to the person is sufficient.
b) For partnership, corporation, cooperative, or joint venture, submit a duly notarized Secretary's Certificate or Board/Partnership Resolution or Special Power of Attorney whichever is applicable.
