REQUEST FOR SUBMISSION OF PROPOSAL

Philippine Council for Agriculture, Aquatic and Natural Resources Research and Development (PCAARRD) is inviting suppliers to participate in a “Small Value Procurement” (Section 53.9 of RA 9184 and its RIRR) for the “Preventive Maintenance of Passenger Elevators at BPK and DPITC Building” with an Approved Budget for the Contract (ABC) of ONE HUNDRED TWENTY-EIGHT THOUSAND SEVEN HUNDRED SIXTY PESOS AND 13/100 (PhP128,760.13).

Submission of proposals including Mayor-s/Business Permit*, PhilGEPS Registration Number*, Notarized Omnibus Sworn Statement (OSS)** and Latest Income Tax Return***, addressed to Dr. Melvin B.
