Description : Preventive Maintenance and Calibration of Multi-Well Gamma Counter Berthold DPC C-12 with SN: 6020 and PN: 2011-05-11-0002-MOLEC at Molecular Imaging & Radionuclide Therapy Unit
Scope of works:
1. Quarterly or Four (4) Preventive Maintenance and Calibration visits for One (1) Year @ PhP30,000/visit
2. Inclusive of Corrective Maintenance Visit
3. Unlimited service calls in case of sustaining repair
4. General functional check-up, system diagnostic, cleaning & lubrication if necessary
5. Must provide stickers on the equipment indicating the date and due date of preventive maintenance and calibration
6. The Service Provider must present a certificate that they are trained and qualified technician/engineer to handle the said equipment
Delivery Terms: Completion of Work shall be on Quarterly or Four (4) preventive maintenance and calibration visits for One (1) Year upon receipt of Notice of Proceed
