Note:
1. All entries must be typewritten or in print.
2. Delivery shall be for a maximum period of forty-five (45) calendar days.
3. Warranty shall be for a minimum of six (6) months for the accessories and one (1) year for the instruments,
from the date of acceptance by the procuring entity.
4. Price validity shall be for a period of forty-five (45) calendar days.
5. (1) PhilGEPS Registration Certificate, (2) valid Mayor-s/Business Permit, and (3) Notarized Omnibus Sworn
Statement shall be attached upon submission of the quotation.
6. Bidders shall submit original brochures showing certifications of the product being offered.
7. Bidders must indicate the BRAND and MODEL NUMBER offered when applicable.

Others:
- Quotations submitted via email shall not be accepted and shall not be
acknowledged in the bid participation.
