4. Duly Notarized Statement of all on-Going Government and Private Contracts
5. Duly Notarized Statements of:
a. At least one (1) Completed Similar Contracts (Government or Private) for the last five (5) years.
b. Single Largest Completed Contract (SLCC) at least 50% of the total ABC.
6. Project Requirements:
a. List of Contractor's Equipment Units which are owned, leased and/or
under purchase agreements, supported by the certification of availability
of equipment lessor/venor for the duration of the project
