Submit your quotation duly signed by you or your duly authorized representative; and, submit copies of the following eligibility requirements not later than May 12, 2025 at 05:00 PM;
1. Valid Mayor-s/Business Permit
2. PhilGEPS Reg. Number
3. Latest Annual Tax Return filed thru Electronic Filing and Payment Systems (EFPS) and must be duly validated with the tax payments made thereon for the preceding tax year be it on calendar or fiscal year income; or
Latest Business Tax Return filed thru EFPS and must be duly validated with the tax payments made thereon, also refers to the Value Added Tax (VAT) or Percentage Tax Returns covering the previous six (6) months.
4. Duly Signed and Notarized Omnibus Sworn Statement
5. Updated/Valid Tax Clearance Certificate as strict requirement for processing of payment.
