INSTRUCTIONS FOR SUBMISSION:

1. All quotations, including the eligibility requirements, must be sealed and submitted either:
- In person at the PCC Procurement Office;
- or through a courier addressed to the PCC Procurement Office
2. Suppliers must ensure that their sealed quotations are properly delivered and received by the Procurement Office ON or BEFORE the specified closing date and time.
3. Quotations submitted after the deadline will not be accepted under any circumstances.

ELIGIBILITY REQUIREMENTS:

A. Certificate of registration from either of the following: DTI, SEC, CDA, DA, Department of Labor and Employment, NCIP
B. Notarized Sworn Affidavit
C. Statement of all completed contracts similar to the Community-based Projects
D. Photocopy or scanned copy of the bank book with complete bank account information
E. Latest Income Tax Return (ITR) for the preceding Tax Year
