Additional Requirements:
A. DENR Accredited Transporter of Hazardous and Non-Hazardous Waste (Provide Copy of Valid DENR Transporter Registration Certificate).
B. Provide Memorandum of Agreement with a DENR Accredited Treater (Provide Valid DENR TSD Registration Certificate and Copy of MOA.
C. Submit Certificate of Treatment/Disposal Issued by The Accredited Treater After the Treatment and Disposal of the Waste.
D. Environmental Sanitation Issued by DOH
E. License to Operate
F. Other Documents Needed for Operation and as Required in the TOR.
