  AT NEW BORN HEARING WITH THE FOLLOWING
  SCOPE OF WORKS INCLUDING REQUIRED PARTS:
  
  I. Replacement of Touch Screen
  II. Assessment of the device for other issues/defects
  III. Cleaning of the Equipment
  IV. System Testing and Commissioning
  
  Note: Cost inclusive of labor Cost and other charges
  
  Conditions:Requirements:
  1. Supplier must secure approved Work Permit & submit complete
  list of names of technicians at EFMS Office prior to repair activities
  2. Submit Service Report/Certification of Equipment Operation upon
  completion of the repair
