I. Replacement of 1 pc. Cable and 1 pc. Plug Pin Connection
II. Repair of Plug Pin Connection.
II. Cleaning of the Equipment.
II. Testing and Commissioning including Calibration.

Note: Cost Inclusive of Labor Cost and other charges.

Conditions/Requirements:
1. Supplier must secure approved Work Permit & submit complete list of
names of technicians at EFMS Office prior to repair activities.
2. Submit Service Report/Certification of Equipment
Operation upon completion of the repair.

Delivery Period: 30-60 Days upon receipt of P.O.
