TERMS AND CONDITIONS
1. Issuance of the dose reports should be 10-15 working days upon the receipt of the used dosimeters
2. The OSLD badges will be shipped to Gov. Celestino Gallares Memorial Medical Center within five (5) working days together with the Statement of Account (SOA) for the next subscription.
3. Monitoring frequency should be of bi-monthly basis.
