This is to certify that items 4, 5, 6 of Purchase Request number 32-25 was first awarded to Velez General Supply Corporation being the supplier with the lowest calculated and responsive quotation but said supplier waived to deliver the items through a letter which was duly received last June 19, 2025. The waived item was then awarded on September 1, 2025 to Verlyn-s Enterprises, the supplier having the second lowest calculated and responsive quotation, per BAC Resolution No. 105-N dated June 24, 2025, viz:
