This is to certify that items 1,4,5,7,8,9 of Purchase Request number 206-25 was first awarded to Verlyn-s Enterprises being the supplier with the lowest calculated and responsive quotation but said supplier waived to deliver the items through a letter through a letter from Mrs. Vergelina B. Caro, Proprietor. The waived item was then awarded on December 3, 2025 to Uymatiao Trading Corporation, the supplier having the second lowest calculated and responsive quotation, per BAC Resolution No. 150-P dated September 16, 2025, viz:
