Description : NOTICE FOR NEGOTIATED PROCUREMENT

The East Avenue Medical Center, through Bids and Awards Committee, invites bidders to apply for the Negotiated Procurement - Small Value Procurement - Surgical C-Arm Software Repair (see attached file for the specification and estimated price.)

Please submit your sealed bids on or before June 16, 2021, 2:00PM with the following requirements:

• SEC or DTI Registration
• BIR Certificate of Registration
• Tax Clearance
• PhilGEPS Certificate
• Audited Financial Statement including Annual Income Tax Return 2019

Only those who submit the complete requirements will undergo review and evaluation.
