• SCHEDULE OF ACTIVITIES:
1. LGU Tunga - July 29, 2025 (500 pax)
2. LGU Tanauan - July 31, 2025 (500 pax)
3. LGU Dagami - August 5, 2025 (500 pax)
4. LGU Palo - August 7, 2025 (500 pax)

Total number of participants: 2,000 pax

*Each packed meal shall include:
• AM SNACK (packed)
1 pc. of Hamburger
1 pc. of bottled juice or soft drinks (at least 250ml)
• LUNCH (packed)
1 rice
1 dish of pork/beef
1 dish of chicken
1 dish of noodles
1 type of dessert
1 pc. of bottled juice or soft drinks (at least 250ml)

*Meals must be delivered to Tacloban Local Health Insurance Office, Brgy. 76 Fatima, Tacloban City on the day of the activity at 7:00AM.

*The date may be changed, depending on availability of speakers and guest. PhilHealth end-user shall inform the supplier at least 5 working days before the schedule.
