Description : Please quote your lowest price inclusive of VAT on the item/s listed below, subject to the Terms and Conditions of this RFQ, and submit your quotation duly signed by your representative not later than 9 December 2025 (1:25 p.m.) at the Civil Service Commission Regional Office VIII, Candahug, Palo, Leyte.

Upon receipt of the quotations on or before the submission deadline, the BAC shall open the quotations.

Supplier shall submit the following requirements:
1. Duly signed Request for Quotation. Prices shall be quoted in Philippine Peso.
2. Mayor-s/Business Permit
3. PhilGEPS Certificate

Please refer to the attached Request for Quotation.

All deliveries by suppliers shall be subject to inspection, and acceptance by the end-user.

The Supplier who submitted the lowest calculated responsive quotation shall be awarded the Purchase Order after evaluation by the BAC.
