Description : 1. The Joni Villanueva General Hospital (JVGH), through its authorized funding source under the through General Appropriations Act (GAA) 2025 intends to apply the sum of TWO HUNDRED FORTY THOUSAND PESOS (PHP 240,000.00), being the Approved Budget for the Contract (ABC) for the lot procurement of meals and snacks for mother and baby friendly hospital initiative: lactation management training for jvgh employee in excess of the ABC shall be automatically rejected.
2. The procurement project consists of lot items and shall be undertaken through Small Value Procurement (SVP) pursuant to Section 53.9 of RA 9184, as amended by RA 12009, and its 2016 Revised Implementing Rules and Regulations.
3. Interested suppliers must submit a duly signed Request for Quotation (RFQ) Form, together with certified true copies of the following eligibility documents:
o Valid PhilGEPS Registration
o Mayor-s Permit
o Tax Clearance
