Description : 1. The Joni Villanueva General Hospital (JVGH), through its authorized funding source under the through Hospital Income Execom Resolution 2025-006 intends to apply the sum of NINE HUNDRED THIRTY THOUSAND PESOS (PHP 930,000.00), being the Approved Budget for the Contract (ABC) for the line procurement for other supplies and materials in excess of the ABC shall be automatically rejected.
2. The procurement project consists of line items and shall be undertaken through Small Value Procurement (SVP) pursuant to Section 53.9 of RA 9184, as amended by RA 12009, and its 2016 Revised Implementing Rules and Regulations.
3. Interested suppliers must submit a duly signed Request for Quotation (RFQ) Form, together with certified true copies of the following eligibility documents:
o Valid PhilGEPS Registration
o Mayor-s Permit
o Tax Clearance
