- With Laboratory Test Result: Negative to Avian Influenza
- With Veterinary Health Certificate signed by Licensed Veterinarian
- With Poultry Handler/Dealer Registration
- With BAI Transport Carrier Accreditation
- Must provide travel plan of delivery with strict compliance to the Provision of Animal Welfare Act (AWA)
- Should be placed in crates during delivery
- Replacement of stock/s in case of downer/mortality within five (5) days after delivery
- With wing band
- Supplier must have a stockyard within the region
