LEARNING SERVICE PROVIDER FOR THE CONDUCT OF DIGITAL LITERACY TRAINING FOR BPI EMPLOYEES

Section I. MANDATORY REQUIREMENTS (Note: Failure to submit any of the following requirements will be automatically disqualified)
a. PhilGEPS Registration (Platinum Membership or Red Membership)
✓ Mayor's / Business Permit
ü Income/Business Tax Return (BIR Form 1701 for ABC 500k and above)
ü Certificate of BIR Registration (BIR Form 2303)
ü DTI/SEC/CDA Registration (whichever is applicable)
● Submit a clear copy or documentation of the proof of payment for the renewal of expired permit/s.
b. Photocopy of one (1) Company ID or one (1) Valid ID w/ three (3) specimen signature
