Layout Services for the Production of IEC Materials for MLGP Cycle 4-Engaged LGUs through Compilation of LGU Health Challenges in Booklet to wit;

Due date: 30 calendar days upon receipt of Purchase Order.
Concept:
Develop a creative concept appropriate to the health challenges and MLGP journey of the
MLGP Cycle 4 engaged LGUs
Graphic Service:
Layout and editorial services for the booklet with an estimated total number of 100 pages,
including its cover page in 8.5" x 11" size; inclusive of articles, high resolution photos; graphs
and charts will acquire from the DOH WV CHD and municipality data.

OTHER REQUIREMENTS/SPECIFICATIONS:
1. End-user may require as many revisions as needed until a design is deemed
acceptable and approved.
2. Ownership of the production and editorial shall be assigned to DOH WV CHD and
shall not be used in other projects without prior consent of the owner
