2. Salary index card - 300 pcs (p25.00/pc)
3. Salary Card Form - 4 reams (p650.00/rm)
4. Leave Form Short - 40 reams (p650.00/rm)
5. DTR - 20 reams (p650/rm)
6. Trip Ticket form long - 40 reams (P650.00/rm)

Note: Items Nos. 1,3,4,5&6 for High Speed Printing using Bondpaper Subs. 20 (as per sample)
