A. QUALIFICATIONS OF SUPPLIERS

1. Only the most competent, responsible and duly accredited suppliers are
qualified to participate

2. To ensure high quality and acceptability of the various nutritional supplies,
interested suppliers shall submit the following for verification:
a. Valid Certificate of Product Registration (CPR) signed and sealed for at
least one (1) year
b. Valid Certificate of Exclusive Authorized Distributor
c. Valid Certificate of Good Manufacturing Practice or its equivalent
d. BFAD Registration

3. The winning supplier shall present an Assurance Guarantee Letter once the
expiration of their delivery of Liquid Nutritional is less than one year.
These shall be replaced two months before expiry date.

B. GENERAL CONDITIONS

1. All Suppliers are required to attend the pre-bid conference.

2. All price quotations per bidded items shall be in typewritten form. Any
corrections made on the price quotations shall likewise be typewritten and
shall be properly initialed befor...
