The Isabela State University Ilagan Campus invites registered contractor/ suppliers/ bidders to submit bid proposals /price quotation of the items listed in the attached RFQ/RFP.
Submit your proposals/ quotation duly signed by you or your duly authorized representative with certified photocopies of the following eligibility requirements is sealed envelope:
a) SEC/DTI/CDA Registration;
b) Business/Mayor-s permit;
c) BIR Certificate of Registration (Form 2303);
d) Certificate of PhilGEPS;
e) Latest Income Tax Return (500k and above);
f) BIR Tax Clearance Certificate;
g) Omnibus Sworn Statement (original copy)

Additional Requirements:

AUDITED FINANCIAL STATEMENT
CERTIFYING BODY MUST BE DULY ACCREDITED BY THE DTI-PAD AND IAF
CERTIFYING BODY MUST HAVE AUDIT EXPERIENCE/ TRACK RECORD IN THE FIELD OF HIGHER EDUCATION FOR ATLEAST TEN YEARS
COST OF CERTIFICATION DETAILS
