SUPPLY AND DELIVERY OF VARIOUS MEDICAL SUPPLIES AND

DEVICES UNDER IB NO. 26-10

1) The Department of Health Bicol Center for Health Development (DOH Bicol CHD),
through the National Expenditure Program intends to apply the sum of Twenty-Four
Million Forty Thousand Six Hundred Three Pesos Only (Php 24,040,603.00) being
the Approved Budget for the Contract (ABC) to payments under the contract for Supply
and Delivery of Various Medical Supplies and Devices under IB No. 26-10. Bids
received in excess of the ABC shall be automatically rejected at bid opening.

2) The DOH Bicol CHD now invites bids for Supply and Delivery of Various Medical
Supplies and Devices under IB No. 26-10 using the Early Procurement Activity.
Delivery of the Goods is required on the expected delivery date stated in Section VI,
Schedule of Requirements. Bidders should have completed, at least two (2) years from
