Before the Permit To Transport (PTT) shall be issued to the HW generator and transporter, HW transporter shall submit to the Airport the following documents:
A. Notarized Memorandum of Agreement/Affidavit of Undertaking/Service Agreement between HW Generator, TSD Facility and HW Transporter
B. Transport Management Plan
C. Schedule of Hauling/Transport of wastes
D. Route of Transport

Certificate of Treatment shall be forwarded to Iloilo International Airport on and before 45days from the receipt of the wastes.
