Description : Please quote your lowest price inclusive of VAT on the item/s listed below, subject to the Terms and Conditions of this RFQ, and submit your quotation duly signed by your representative not later than
25 July 2023 (12:01 pm) at the Civil Service Commission Regional Office VIII, Candahug, Palo, Leyte. For more information, Please call 053 888-0742 or email at ro08.msd@csc.gov.ph.

Upon receipt of the quotations on or before the submission deadline, the BAC shall open the quotations.

Supplier shall submit the following requirements:
1. Duly signed Request for Quotation. Prices shall be quoted in Philippine Peso.
2. PhilGEPS Registration Certificate
3. Valid Mayor's/Business Permit
4. Notarized Omnibus Sworn Statement

All deliveries by suppliers shall be subject to inspection, and acceptance by the end-user.

The Supplier who submitted the lowest calculated responsive quotation shall be awarded the Purchase Order after evaluation by the BAC.
