NOTE: Each BOD and employee shall be entitled to one (1) eye examination with eyeglass package not exceeding Three Thousand Pesos (3,000.00), inclusive of the following:
* Professional eye check-up fee
* Prescription lenses
* Eyeglass Frame
Any add-ons or upgrades exceeding P3,000.00 budget shall be shouldered by the concerned BOD or employee.
