3. Product insert/product information
4. Certification/Agreement/Contract between the bidder and the Manufacturer/Distributor/Importer/
Wholesaler, as specified in the CPR, that the bidder is an authorized dealer or distributor of the item to be bid
5. Certificate of Compliace to the Electronic Drug Price Monitoring System (EDPMS) issued by either
the DOH Pharmaceutical Division or the DOH Regional Health Office/Centers for Health Development
6. Supplier to submit the FDA Result of Analysis, if applicable
