2. Copy of Sanitary Permit from the City/Municipal Health Office.
3. Copy of Environmental Sanitation Clearance (ESC) from the DOH.
4. Copy of Pollution Control Officer (PCO) Accreditation issued by DENR-EMB or LLDA.
5. Copy of MOA with Septage Treatment and Disposal Facilities, if applicable.
6. Copy of Environmental Compliance Certificate (ECC) of the Septage Treatment and Disposal Facilities.
7. Copy of Waste Water Discharge Permit of the Septage Treatment and Disposal Facilities.
8. Accomplished Manifest Form.
9. After the Treatment/Disposal of the Waste, Certificate of Treatment from the Accredited Treatment Facility and Certificate of Disposal from the Disposal Facility must be submitted
