Description : INVITATION TO NEGOTIATE

Dear Sir/ Madam:

Due to two (2) failed public biddings for Circular Proposal No. CP 2025-73 for the Supply and Delivery of IEC Supplies c/o CDRRMO, may we request your good company to submit the attached Request for Quotation No: CP 2025-73 Negotiated Procurement together with the following requirements:

1) Request for Quotation (Attached hereto);
2) Complete brands and Technical Brochures/Pictures w/ specifications of Offer;
3) Valid PhilGEPS Certificate of Registration (Platinum Membership) in accordance with Section 20.2.9.1 of the IRR of RA No. 12009;
4) Original Omnibus Sworn Statement and if applicable, Original Notarized Secretary-s Certificate in case of a corporation, partnership, or cooperative;
5) Valid Mayor-s/Business Permit;
6) Latest Income and Business Tax Returns; and
7) Latest Audited Financial Statements.
