*Specifications:

1. The Service Provider must be able to provide the following:
a. Updated Mayor-s/Business Permit
b. HilGEPS Registration Number
c. Proof of Updated PhilHealth Contribution
d. Updated Busines/Income Tax Return
e. Updated BIR Tax Clearance

2. The Service Provider must have at least eight (8) hours of business operation and must have branches in the following areas with PhilHealth offices:
a. Tacloban City, Leyte
b. Ormoc City, Leyte
c. Baybay City, Leyte
d. Naval, Biliran
e. Maasin City, Southern Leyte
f. Calbayog, Samar
g. Catbalogan, Samar
h. Catarman, Northern Samar
i. Borongan, Eastern Samar

3. The Service Provider must acknowledge the goods or items received through an Acknowledgement Receipt to be provided by PhilHealth.

4. The Service Provider must have a readily accessible website through internet for monitoring/tracking of packages through its tracking number.
