  2. Price validity shall be for a period of 90 calendar days.
  3. Please submit the following documentary requirements upon submission of the quotation.
  a. Mayor-s/ Business Permit
  b. PhilGEPS Registration Number
  c. Business/ Income Tax Return (for ABC-s above 500k)
  d. Notarized Omnibus Sworn Statement shall be submitted once awarded to the winning bidder (for ABC exceeding 50K)
  4. Philgeps Reference number should be written outside the sealed bid/quote envelope.
