3. Valid and Current Mayor-s/Business Permit 2025;
4. Tax Clearance Certificate / BIR Certificate of Registration;
5. Original and Notarized Omnibus Sworn Statement (OSS).

Award of contract shall be made to the lowest calculated and most responsive quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form in “Annex B” and in the Technical Specifications which lists the details of the requirements, as indicated in “Annex A".
