1. Mayor's/Business Permit;
2. PhilGEPS Registration Number/Certificate;
3. Omnibus Sworn Statement*;
4. Business/Income Tax Return; and
5. PCAB License

Submission of Quotation will be on or before 9:00AM of December 23, 2025.
Opening of Quotations will be held on PENRO Conference Room, 10:00 AM on December 23, 2025.
