TERMS & CONDITIONS:
1. All entries must be typewritten.
2. The following documents must be attached with the quotation:
a. Valid and current Mayor's permit/Business Permit
b. DTI/SEC Registration
c. Updated General Information Sheet
d. Annual Income Tax Return/Business Tax Return
e. Tax Clearance Certificate
f. PhilGEPS Certificate of Registration-Platinum Member
g. PCAB License
3. Delivery/Completion must be within sixty (60) calendar days upon receipt of Notice to Proceed
4. Payments shall be on a staggered basis
5. Quotation validity shall be for a period of at least thirty (30) calendar days.
6. Quoted price must include all applicable taxes and duties
7. Award will be made to the Lowest Calculated and Responsive Quotation.
8. Jaen Water District shall reserve the right to post-qualify and/or to reject all submitted quotations.
