TERMS OF REFERENCE

Project Title : Conduction of Pest Control Services to Various Areas (Termites, Rats, Cockroaches)
Location : Cagayan Valley Medical Center (OB Ward, Dietary, Psych Ward and Autoclave OR)

Termite Control Application

1. Application of bait or chemical to terminate in the area
2. Monthly checking shall be done to ensure the termination of termite for one year
3. Excavation of Termite mount shall be done if possible

Rats and Cockroaches and other flying insects
1. Application of harmless or chemical to eliminate Rats and Cockroaches and other flying insects in the area
2. Monthly checking shall be done to ensure the termination of rats and cockroaches for one year
3. Carcass shall be properly disposed outside hospital premises
