Description : ANNUAL PHYSICAL EXAMINATION TERMS OF REFERENCE (TOR)

In line with the conduct of the 2025 Annual Physical Examination, the SBCorp Regular Plantilla Personnel who are on board as of May 22, 2025, following are the details of the Terms of Reference (TOR).

I. APPLICABILITY

The Terms of Reference (TOR) is applicable to all SB Corp personnel in both Head and Area Offices.

II. COVERAGE AND DELIVERABLES

Consistent with previous years, the 2025 APE will cover the following medical examinations and laboratory tests:

• Basic 5
o Full Medical Examination (e.g., physical exam, medical history, visual acuity),
o Complete Blood Count,
o Chest X-Ray,
o Urinalysis, and
o Fecalysis
• Blood Chem Test
o Fasting Blood Sugar (FBS),
o Lipid Profile, and
o Blood Uric Acid (BUA)
• Other Test (for employees 35 years old and above)
o ECG
o Papsmear

B. Provide at least 2 doctors (male & female) during the conduct of APE
