1. The University of the Philippines - Philippine General Hospital (UP-PGH), through the PGH Revolving Fund CY2025 intends to apply the sum of TWO MILLION NINE HUNDRED EIGHTY THOUSAND PESOS & 00/100 (PHP2,980,000.00) ONLY, being the ABC to payments under the contract for the SUPPLY, DELIVERY AND TESTING OF ONE (1) SET OF BRAND NEW TWO-CHANNEL RADIOFREQUENCY LESION GENERATOR FOR THE DIVISION OF NEUROSURGERY, DEPARTMENT OF NEUROSCIENCES, PHILIPPINE GENERAL HOSPITAL, under Project Reference No. BAC-2025-08-0642.
