1. The University of the Philippines - Philippine General Hospital (UP-PGH), intends to
apply the sum of SIX MILLION TWO HUNDRED THOUSAND PESOS AND 00/100
(PHP6,200,000.00) ONLY through Fund 164, inclusive of all taxes, such as, but not
limited to, value added tax (VAT), income tax, local taxes, and other fiscal levies,
being the ABC to payments under the contract for Supply, Delivery and Testing of
One (1) Lot Lithotripsy System compatible with Existing Equipment (Storz
Nephroscope) for the Division of Urology, Department of Surgery, Philippine
